Follow-up for New Patient Package 

Both the New Patient Package and The Personalized Treatment Plan Package include a free follow-up appointment to review progress with your personalized Cannabis plan. These appointments are typically scheduled 4-8 weeks following the initial Treatment Plan appointment. A brief intake form is completed upon scheduling and will be reviewed in advance of you appointment. You appointment will  include up to 15 minutes of consultation with the physician and adjustments to the treatment plan as needed. You can schedule on-line.
